Jersey Assistance for Community Caregiving (JACC) Social Worker – Part Time
REPORTING RELATIONSHIP
Reports to the Director of Community Support Services
SUMMARY
Responsible for assessing applicant’s needs, coordinating services, and providing ongoing case management to JACC clients.
RESPONSIBILITIES
Within the limits of the agency’s policies and procedures, a social worker is responsible and has commensurate authority to perform the duties listed below and is directly accountable to the Director of Community Support Services. Performance of these duties requires extensive and regular supervision. Duties included are:
- Making initial contact with applicant within 3 business days of case being assigned.
- Conducting first home visit within 7 business days of case assignment.
- Completing a psychosocial assessment during the home visit to determine applicant’s needs.
- Developing a plan of care (POC) based on client’s assessed needs and program requirements; and obtaining all signatures within 30 calendar days of case assignment.
- Utilizing POC to arrange both formal and informal supports.
- Ongoing case management is provided with a minimum of a monthly contact, quarterly visit and an in home visit every 6 months.
- Participant services are authorized through Individual Service Agreements (ISA), Special Requests, and Prior Authorizations.
- Ongoing care coordination is provided with service providers.
- Monitoring records are kept ensuring all services are being provided in appropriate time frames.
- Maintaining service cost cap in Service Cost Record and staying in the client’s budget.
- Notifying appropriate agencies of case disenrollment/ withdrawal and transfers.
- Providing Director with monthly reports as requested.
- Attends a minimum of 1 continuing education training related to the elderly or physically disabled annually.
- Access to clinical record information is limited to the extent required to perform responsibilities effectively & efficiently.
- Responsible for maintenance of client records as required by grantors.
- Attends bimonthly department meetings as required by Director
WORKING RELATIONSHIPS
Works closely with the Director of Community Support Services, social workers, and agency staff.
WORKING ENVIRONMENT
This job operates in a professional office and field environment. Exposure to unsanitary conditions, vermin and insect infestation, client bodily fluids, and clients who maybe infected with TB, AIDS, or infectious illnesses. Field visits are sometimes made to condemned houses or homes with various types of structural damage. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. PHYSICAL DEMANDS
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. Majority of work is of light to medium duty, which requires standing, sitting, bending, speaking, hearing, manual dexterity and visual acuity. Use of a personal vehicle and the ability to drive a car is expected.
BACKGROUND/EDUCATION/TRAINING/EXPERIENCE
- Bachelor’s Degree in a health related or behavioral science field required.
- Must have at least one year of experience working with the elderly or physically disabled in an institutional or community setting.
- Have demonstrable work habits, such as accountability, punctuality, and respect for clients.
- Have the ability to accept and utilize supervision.
- Excellent communication and organizational skills; ability to focus on details.
- Competency in communication, e.g. reading, writing and speaking
- Must be computer literate.
- Must have a valid New Jersey driver’s license and an available, insured, motor vehicle.
